Our facility is street level, handicapped accessible, and has available street parking. Our office meets and surpasses all OSHA (Occupational Safety and Health Administration) and CDC (Center for Disease Control) standards. New Patients
Prior to your first appointment, please print and complete the new patient forms found on this website under the Forms tab. A complete dental examination will be performed which will include an evaluation of the teeth and oral soft tissues, oral cancer screening and appropriate x-rays/radiographs. If you have had recent x-rays/radiographs taken you may bring them with you, or have them forwarded to us. Following the complete dental exam, we will present a treatment plan designed to improve your dental health.
Financial
We will be more than happy to submit all insurance forms and help you recover the most from your benefit plan. We are members of Delta Dental Insurance Company. For patients requiring extensive dental work, a complete payment plan can be designed with an appropriate payment schedule. Forms of payment accepted by the office are cash, check, or any major credit card. You can also pay your dental bills online using this website.
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient.
